Q:Claim was rejected Hi, I purchased medical insurance from insubuy for my in laws'. We had to visit the doctor cause my mother-in-law got swelling on her face. The doctor prescribed her prednisol. I paid my copay and was expecting the insurance covers the rest. but I got a letter from the Insurance company that they are rejecting the claim. Reason code 45. description condition not covered under plan. where can I get a comprehensive list of conditions not covered?

A:
I am sorry to hear that you are having difficulties with your claims. The details of covered benefits and exclusions can be found in the certificate wording that you would have received when you purchased the insurance and it is also available on this web site. However, you would have to specify which product you purchased. Please call us so that we can find out more details about your purchase and and also about the condition. If you are having legitimate issues, we would be glad to help you with your claim. We purchased the Visitor Secure Plan. The doctor's office has already send the bill to you. Do I need to send you any document other than that. The last date to pay the bill is 03/23/15. From your last post, it is not clear whether the claim was actually rejected or it is pending. Please note that Insubuy does not process any claims. Therefore, for regular day to day matters, you would work with the insurance company's administrator as we have no information about it. That contact information is given on your insurance card. However, if you are having difficulties with your claim, we would certainly be able to help you. But for that, you would have to call us and provide the details so that we can analyze your situation.

Q:Atlas America and its PPO network Does the PPO network agreement with the in-network medical providers limit the price that the provider can charge for a specific service? Another way to put it is: does the PPO network agreement reduce the amount the provider can bill, if the provider charges too high (which would show up as a PPO discount of the services on an explanation of benefits statement)?

A:
Yes, Atlas America participates in Coventry/First Health PPO network. Yes, when you visit the providers within the PPO network, they will charge only the network negotiated fees and you don't have to pay the difference if they charge too high. You can purchase it online at /atlasamerica-insurance/ Please let us know if you have any other questions.

If so: 1. do you send it simultaneously with whatever the medical provider does, or do you wait until the medical provider actually sends you a bill? 2. do you have to send the invoices and all the travel documents each time?

A:

Q:health insurance premium for H4 when in India for few months Hi, I am on H1B and my wife on H4. I have health insurance through my employer and I have to pay a big premium for both of us. Now my wife went to India for 3-4 months and I would like to stop paying premium for her(it is expensive) until she comes back by removing her from the insurance. But my employer is saying it is not possible as you cannot remove/add without any life changing event like divorce, marriage etc...? Please clarify. Is there anyway I can remove her from the insurance, buy a short term insurance for her until she comes back, then cancel that and then add her back to my employer insurance?

A:
Your employer is right. You can NOT enroll in and out of major medical (long term) plan whenever you want. Long term plan means you keep it year round. If you don't, you may be subject to financial penalties as well.

Q:Insurance Presale Questions My mother is coming to visit me in USA for 74 days and I am planning to take a visitor insurance. I have a few questions. 1 My mother had breast cancer 12 years back and it is completely cured now. Can she take insurance from your website? 2. Indian insurance companies give emergency advance, baggage loss ,loss of passport etc. Do you provide the same? 3. Do you have to notify the insurance company in advance if you want to visit the doctor? Thank you Sunny

A:
Thank you for your interest in purchasing visitors insurance for your mother. 1. Yes. Please make sure to read the benefits of each plan as it varies. 2. Some of the plans we offer cover loss of checked luggage or loss of passport etc. However, please note that those are small benefits on the side. Your main concern should be to get proper medical coverage that is easy to use and adequate when you really need it. Please read /visitors-insurance-usa-india-comparison/ to understand the difference between buying from US v India. 3. No. Not for the doctor visit but you do have to notify for major things like surgery, hospitalization etc. That is called pre certification or pre notification. Please read /visitors-insurance-precertification-requirements/ for more details. It would be the easiest if you could kindly call our office and we can go over various plans to find the most suitable one for you.

Q:Question about age Can someone clarify the age requirements listed when obtaining quotes? Is this the age at the time the insurance policy is bought/start date, or the age at the end-date of the policy? If someone I am buying the policy for turns 70 during the coverage period, what is the age I should list when buying the policy?

A:
It is the age as of the effective date. And just to be clear: the effective date is the start date? Yes
