The coverage would not change for your mother when she goes from 59 to 60 but the premium would increase with most plans upon renewal. For example, if you purchase for one month you would pay the premium for a 59 year old. If you renew the policy the next month you would then be charged a higher premium based on the current age of 60. If you instead purchase the plan for the full duration then that would lock in the premium for a 59 year old for the full duration. She would not have to change plans at any point. As far as billing, it is at the providers discretion whether or not they would like to bill directly or have you pay out of pocket.
