The plans do not cover anything preventative, routine or pre-existing. Anything that an insured already has, known or unknown at the time of application would not be covered by any plan. Some plans offer an acute onset benefit. If someone has an acute onset of a pre-existing condition that meets the definition provided within the plan's certificate wording, it could be covered based on the doctor's or attending physician's report. The acute onset is a possibility of coverage in the case of an acute onset.
