Q:I have a few questions about the quote I received from your website. 1. Can you provide me with the network hospitals in Miami Florida for the Atlas America Insurance? 2. What is considered "urgent care" ? 3. Is "urgent care" an emergency visit? 4. I noticed "urgent care" does not require a $250 deductible but an "emergency visit" requires a $250.00 deductible plus a coinsurance payment. 5. What is the coinsurance payment? 6. What is the monthly premium on the $493.68 total price? 7. What happens after the first 5,000 medical coverage is used up? Example: For any subsequent treatment (whether for the same condition or a different condition) you do not have to pay the deductible again. If the provider were out of the network and/or they didn't bill the insurance company directly the insurance company will continue to pay 80% for the first $5,000 of coverage medical expenses you continue to pay 20% ( that is maximum $1000). After that the insurance company will pay 100% for covered medical expenses. Up to 50,000.00. 8. What is considered "covered medical expenses after the first $5,000"?

A:

Please read how plan works. 5. Co-insurance is the percentage shared between insured and insurance company 6. Premium will be depends on what deductible and policy max. you choose but it .
