Q:I want to buy insurance for my family and have some queries regarding filling the first and last of the applicants. So the problem is: My sister doesn't have Last Name in her passport and her first name is Shivani. Therefore, as per her passport the name is registered as follows: Given Name: Shivani SurName: i.e Surname is blank, On her US B1 Visa the name is mentioned as: Surname: Shivani Given Name : FNU So my query is what should I put in first name and last name for my sister? The following criteria are required: 1. Medical Benefits of at least $100,000 US dollars per accident or illness. 2. Repatriation of remains in the amount of $25,000 US dollars (Definition of Repatriation: If a person dies in a foreign country, we say they are "repatriated" when their body is returned to the native country. Repatriation insurance is used for transportation expenses related to returning the deceased to their native country.) 3. Expenses associated with the medical evacuation of the non-immigrant student to home country in the amount of $50,000 US. 4. Deductible not to exceed $500 per accident or illness. 5. An A.M. Best rating of ''A-'' or above; 6. McGraw Hill Financial/Standard & Poor's Claims paying Ability rating of ''A-'' or above; 7. Weiss Research, Inc. rating of ''B+'' or above; 8. Fitch Ratings, Inc. rating of ''A-'' or above; 9. Moody's Investor Services rating of ''A3'' or above. Does the Atlas Essential America Insurance cover all of the above criteria? Many thanks.
