So the problem is: My sister doesn't have Last Name in her passport and her first name is Shivani. Therefore, as per her passport the name is registered as follows: Given Name: Shivani SurName: i.e Surname is blank, On her US B1 Visa the name is mentioned as: Surname: Shivani Given Name : FNU So my query is what should I put in first name and last name for my sister? When completing an application if only the first name is given in the passport you will list the first name in the given filed and the last name as LNU. Q:I will be in the US from September 17, 2107 to April 30, 2018 in an exchange doing research in Stanford University. I need to get health insurance for this time and it needs to fulfill the requirements listed in the attached International Student Insurance Coverage Certification Form. Looking at your website, I saw the Patriot Platinum America on the section for exchange visitors with a J-1 visa. To me, it looks more generous (mainly because it has a 5 million USD limit) than the Student Secure ELITE Plan you suggest and is also cheaper. Since I am no expert, I would like to know why you recommend the Student Secure ELITE Plan?
