I understand that the amount would be 636$ for the 10 months starting from March 1st, 2016 through December 31, 2016. Yes, she is under an F2 visa. I also understand that she will have to pay a first deductible of 100$ per incident. Is that applied for a routine medical/dental check with a pediatrician for example?
