Q: In Atlas America, After deductible, pays 100% (URC out of network) to policy maximum, what do you mean by "URC out of network"?

A:
"URC" are the Usual, Reasonable and Customary charges in the area from which services are provided. If an individual goes outside of the PPO network and received treatment there are no network negotiate rates that are offered. Going out of network the provider is going to charge their standard rates for the services rendered. When this is done the plan will only cover what is considered to be a standard rate in the area that service were render in. So for example, if a charge of $1,000 was issued for an out of network expense and the URC in the area is $600 the plan will only pay $600 for that claim. Q:Does getting the Diabetes (sugar test) test to check the sugar levels come under acute on set for pre existing condition? All I wanted is to have a doctor examine to get the blood work done to test the sugar levels.

Unfortunately, there is no insurance for an international visitor which covers routine maintenance for the pre-existing conditions. You have to pay from your pocket. Q:I read about all the details related to the plan but was not sure about two matters. 1. Once deductible is paid, their expenses in UK will be covered at 100%? 2. I read couple of experiences of buyers with Patriot plus where they mentioned pre-certification matter. I read about it a bit but it was not clear. Can you please tell me about it in detail?

A:
Outside of the United States and Canada, eligible medical expenses after the deductible has been met would be covered at 100% to the policy in accordance with the certificate wording. Precertification is required for any for the mentioned medical service in the certificate wording such as artificial limbs, CAT scans, Durable Medical Equipment, any treatment in and Extended Care Facility Inpatient, any Home Nursing Care, Inpatient status, MRI or any Surgery or Surgical procedure etc. If any of these scenarios happen the insured person or his/her physician or healthcare provider must contact the claims department within 48 hours or as soon as reasonably possible or you may be subject to additional financial responsibility for not having done so. Q:My parents are visiting USA and UK and I was wondering if I can buy Patriot America Plus. The plan details shows that it covers expenses incurred in USA. Will it cover expenses incurred in UK as well?

Q: I am a Chinese citizen. I am going to travel to Canada and U.S. I will stay in Maryland most of my time. When I look at your Atlas America plan, there's a geographic restriction for Maryland. My question is, will the insurance cover my family when we stay in Maryland?

The restriction states that the proposed insured cannot be physically located in the country of Canada or the states of Maryland at the time of application. As long as the plan is purchased prior to the arrival into these location you and your family will be eligible for the plan.
