For the difference in t he premium my question is regarding the sudden jump in my mothers premium from 100k coverage to 500k whereas my fathers coverage increase by less amount. Most of the time if you go in the ppo network provider billed directly but some time for dr. visit you may need to pay upfront but if person needs major hospitalization or major expense you don’t need to pay upfront.
