I want to buy insurance for my family and have some queries regarding filling the first and last of the applicants. So the problem is: My sister doesn't have Last Name in her passport and her first name is Shivani. Therefore, as per her passport the name is registered as follows: Given Name: Shivani SurName: i.e Surname is blank, On her US B1 Visa the name is mentioned as: Surname: Shivani Given Name : FNU So my query is what should I put in first name and last name for my sister? Let me know if you have any other queries.
When completing an application if only the first name is given in the passport you will list the first name in the given filed and the last name as LNU.
We need to go according to her passport as this will be her identification while in the United States.
