My mother who is visiting us in USA fell down on the sidewalk and got an abrasion on chin. So, we went to the urgent care to get a tetanus shot for her. The urgent care charged 70 for the shot instead of 30 copay as mentioned on our First Health insurance. Please let me know which form to use for filing the claim.
