I am an Italian citizen and I will be a student at Berkeley University the next semester from January 15th 2019 till May 20th 2019 with a F1 visa. I need a insurance policy that comprehend all of these points : A minimum of $500,000 in benefits 100% coverage of hospitalization fees 100% coverage of professional fees At least 15 days of inpatient mental health services No pre-existing condition exclusion Policy written in English with benefits expressed in US dollars Payment of at least $50,000 for medical evacuation Payment of at least $25,000 for repatriation of remains A network provider facility within 50 miles of the UCB campus (Berkeley ) An annual deductible of no more than $500 Please contact me if you have any insurance that corresponds to these points
