I want to know plans that cover J1 insurance plans which covers following criteria. And I require a J1 heath insurance verification form- how can I get that. 1. Medical benefits of at least $100,000 (US$) per accident or illness; 2. Repatriation of remains in the amount of $25,000 (US$); 3. Expenses associated with the medical evacuation of the exchange visitor (or accompanying spouse or dependent children) to his or her home country in the amount of $50,000 (US$); 4. A deductible not to exceed $500 per accident or illness.
