Hello, I will be a J1 visiting scholar in University of Illinois Urbana-Champaign between July 22 2019 and July 31 2020. I was looking for a health insurance for myself and my two dependents. Looking at your website, I found that your Exchange Guard Plan with 0 deductible meets the J1 visa health insurance requirements. However I have few questions that need to be answered quickly since I will be departing to the US on coming Sunday. 1. Does your ExchangeGuard Choice and Patriot Exchange Plans have the same coverage in terms of outstanding restrictions? Although myself and my dependents do not have any mentioned restriction conditions explained n Exchange Guard Choice plan, I want to be clear on this because as far as I see Patriot Exchange plan does not have a list of restrictions. 2. Do you have a plan that does not have restrictions and does not have waiting period? 3. If I have an injury related to a broken bone or a cut and have 0 deductible plan and 100/0 co-insurance pay, do I have to pay anything to the hospital? 4. During my stay period, if I have an inner ear infection or a stomach ache and have to visit doctor's office, do you require medical history report before paying the claim? If yes, where can I find the forms to be filled for the report? 5. If I purchase the plan for a year at the beginning and they cancel it 30 days later, do I incur any penalty fees or do I get full refund back for the remaining insurance period?

2. No. For that you need to purchase insurance from your school.

3. If you go in the network plan will pay 100%. For Dr. visit you may need to pay upfront but hospitalization or major expense you don't need to pay from your pocket.

4. Yes. Usually attending physician will ask some questions and send medical notes to claim dept. and based on attending physician statement will be determined whether claim dept. needs additional medical records or not.
