Insurance claim is not being processed Hi, I had Atlas America Health insurance. I visited a Urgent care center on 5/22/2014. As I am not aware that i have to send a pre-auth form after the treatment date and i didn't receive any email asking about it from the insurance company, i didn't send the pre-auth form immediately to the insurance company. The insurance company has closed the claim stating the same reason after 3 months. After i learnt the reason from the hospital, i have sent the pre-auth form to the insurance company multiple times and i have also received the confirmation from them through email. I have called the company thrice to request them to reopen the claim. They have said that they are going to put a request and my claim will be reopened in 3-4 days. But every time I call them they say the same old story that it has not been reopened and they will reopen it in 3-4 days. Despite my continuous efforts, the claim has never been reopened and i had to pay the amount to the hospital. Please let me know what is it that i could do to make it processed. Thanks, Vinay
