Dear Sir or Madam, I am a postgraduate student from Poland and I will be visiting the Stanford University as a Visitor Student Researcher (arriving with J-1 visa). I would like to purchase an insurance policy for the six-month internship in US. There are specific requirements and all of them need to be fulfilled. Moreover, insurance company needs to sign a compliance form. Could you please provide me with some plans that meet all the requirements? Insurance policy must cover the entire period of November 1 through April 30 with benefits that meet or exceed the following minimum standards: - An annual deductible less than $1,000 USD (I'd prefer less or no deductible) - Lifetime aggregate maximum benefit of at least $2,000,000 USD OR a maximum per condition/per lifetime benefit of $500,000 USD. - Coverage for inpatient and outpatient medical care in the San Francisco Bay Area and the U.S. - Coverage for inpatient and outpatient mental health care in the San Francisco Bay Area and the U.S. - Prescription drug coverage. - Coverage for non-emergency as well as emergency care. - Coverage for pre-existing conditions or you have met applicable waiting periods. - Minimum coverage of $25,000 USD for repatriation of remains and $50,000 USD for medical evacuation. - The policy must be backed by the full faith and credit of your home country government or the company providing the insurance must meet minimum rating requirements established by DOS (an A. M. Best rating of "A-" or above, an Insurance Solvency International, Ltd. (ISI) rating of "A-" or above, a Standard & Poor's Claims-paying Ability rating of "A-" or above, or a Weiss Research, Inc. rating of B+ or above).
