Medical insurance for Pratt student Hi I am Pratt student and I purchase the health insurance from your company as I want to waive my Pratt 's health insurance ,I need to go online and waive . In the process of internet I need to fill up the form ,but I don't know how to do such as 1.insurance company name. I only found the United healthcare on the internet ,so I don't know what is exactly the name of your company. 2.Type of Insurance Plan as far as I known ,I took Plan A ,but I still can't find any plans are related to my health insurance . 3.Policy of Group Number 4.insurance Plan Name 5.insurance company address 6.insurance Phone Number 7.What is your policy deductible 8.If you are on your paren't plan ,at what age will you no longer be eligible .
