J1 and J2 Insurance questions Dear Sir, dear Madam, I am on J1-Visa (started August 1, 2015, country of provenience: Italy, country of residence: Germany) as visiting assistant Professor at the University of Notre Dame in Indiana and my family (wife, 34, two daughters, respectively 4 and 2, all on a J2-Visa, all with German passport and resident in Germany) accompanies me. Until April 30 we will be covered by the medical insurance of the University of Notre Dame. However we will stay until August 15, so we are looking for alternatives to COBRA from May 1 to August 15. The compliance form of UND states the following requirements for a medical insurance: I understand that the Federal Regulations governing the J-1 visa program require that J-1 visa holders and their J-2 dependents carry insurance coverage that meets certain requirements which are set out in the federal regulations. At least $100,000 per person per accident or illness; at least $25,000 for repatriation of remains; at least $50,000 for medical evacuation to the home country; and a deductible not to exceed $500 per accident or illness. Co-insurance may not exceed 25% payable by the Exchange Visitor or sponsor. Acceptable coverage may not exclude risk inherent in the activities of your Exchange Visitor program. The company providing the insurance coverage must have an A.M. Best rating of ?A-? or above, an Insurance Solvency International, Ltd., (ISI) rating of ?A-1? or above, a Standard & Poor?s Claims-paying Ability rating of ?A-? or above, or a Weiss Research, Inc. rating of ?B+? or above; or backed by the full faith and credit of the government of the exchange visitor?s home country; or offered through or underwritten by a federally qualified Health Maintenance Organization (HMO) or eligible Competitive Medical Plan (CMP) as determined by the Health Care Financing Administration of the U.S. Department of Health and Human Services. We have entered the U.S. on August 12, 2015 and are currently insured with MERITAIN Select PPO, which is the health insurance partner of the University of Notre Dame. It is a family insurance. I am 34 years old, my wife too, our children are 4 and 2 years old.We would need an insurance for the period May 1- August 15, 2016

I'm sorry but there a point that I do not understand...When I get a quote, is the price shown (in my case for example ca. 574 USD) the monthly premium or the total cost of the insurance from May 1 to August 15? For fun I want on with the application up to the payment form and it was the total due.
