Further information on Global Medical Insurance Gold Option Dear IMG, After reading your website information about the Global Medical Insurance plans that you offer, I was hoping you could provide me with further information: I am looking for a global/international family policy for my wife and myself. We are both US citizens who have lived in Hong Kong for the past 9 years and anticipate doing so indefinitely. Specifically, I am looking for a policy that will qualify as a "HDHP" High Deductable Health Plan for the purpose of establishing a HSA Health Savings Account in the US. The requirements for a HDHP are: Health insurance plan with minimum deductible of (for 2007): â€" $1,100 (self-only coverage) â€" $2,200 (family coverage) â€" These amounts are indexed annually for inflation • Annual out-of-pocket (including deductibles and co-pays) cannot exceed (for 2007): â€" $5,500 (self-only coverage) â€" $11,000 (family coverage) â€" These amounts are indexed annually for inflation Reasonable benefit designs not counted toward the out of pocket maximum include: â€" Lifetime limits on benefits â€" Limits to usual, customary and reasonable (UCR) amounts â€" Limits on specific benefits • Maximum number of days or visits covered • Maximum dollar reimbursements â€" Pre-certification requirements • HDHPs can have: â€" first dollar coverage (no deductible) for preventive care (copays allowed) â€" higher out-of-pocket (copays & coinsurance) for non-network services • All covered benefits must apply to the plan deductible, including prescription drugs If HDHP provides prescription drug benefit, prescription drug expenses must be subject to the annual deductible or the individual may not contribute to an HSA. Preventive care generally does not include any service or benefit intended to treat an existing illness, injury or condition. • Certain drugs and medications can be considered preventive care. â€" Drugs taken by a person who has developed risk factors for a disease that has not yet manifested itself or to prevent reoccurrence of a disease â€" Example: Cholesterol-lowering medication for those with high cholesterol Safe harbor list of preventive care that HDHP can provide as first-dollar coverage before minimum deductible is satisfied: â€" Periodic health evaluations (e.g., annual physicals) â€" Screening services (e.g., mammograms) â€" Routine pre-natal and well-child care â€" Child and adult immunizations â€" Tobacco cessation programs â€" Obesity weight loss programs • Can apply co-pays to preventive care services The benefits and exclusions described for your Global Medical Insurance Gold Worldwide coverage policy would seem to be what I am looking for. My concern was about the deductable where it states that the family deductable is "3X the single". I would want the highest deductable allowable under the guidelines for a HDHP provided above. The Global plans are international medical plans. The HSA compatible plans are special plans created specifically for HSA purposes, following certain guidelines, and are domestic plans. There are certain requirements that the Global does not meet (out of pocket limit: for example $5,000 individual out of pocket maximum annually) due to exclusions or having benefits capped off at a specific dollar amount.
