Policy is part of a health benefits program offered on a group basis to employees or enrolled students by a designated sponsor OR ?? Policy is offered through or underwritten by a federally qualified Health Maintenance Organization or eligible Competitive Medical Plan as determined by the U.S. Dept. of Health & Human Services. The minimum coverage requirements stated above are provided by this policy/plan. I am qualified to make this determination as an authorized agent/employee of the above insurance provider.

Answer:

Everything else in the certificate looks ok except pre-existing conditions. Pre-existing conditions are not covered. In other words, after no matter what duration or waiting period, pre-existing conditions are still excluded.
