If I buy fixed coverage plan, can I go to any doctor/ hospital or there is specific list of hospitals that are covered within the fixed coverage plan? What is the PPO network? If a particular hospital is not in the PPO network, then does that mean I have to pay the charges upfront and then file the claim with the insurance company?
