I would like to buy medical insurance for my father as he is visiting the United States next week. I would like to understand the requirements/conditions for me to serve as his proxy? Is a verbal consent okay? Or do I need his written authorization on paper/email? Or do you need an affidavit for the same?
You don't need to sign any of the documents while purchasing insurance for him as a proxy. But in future if he has any claim and you need to discuss about his medical conditions then you need to sign authorization form.
