Hi, I am going to visit USF as a visiting scholar in the mid May. I will apply J1 VISA FOR MYSELF and J2 VISAs for my wife and my 5-year old daughter. They will arrive in US two months later.
I have viewed some insurance plans on your web-site and I am interested in two of them, Patriot Exchange Program and Inbound USA. My questions are:
1. Are these two plans meet the insurance requiremnent of USF?

2. Which one will be more suitable for me and my family members as J1 and J2 status?
3. The co-insu of these two plan is ?%
4. When I purchase a annule plan for 12 months, if this plan will cover my dependents automatically? Or we should buy a seperate insurance for my daughter and my wife?
5. Can we buy a insurance month by month for my daughter and wife, because they will stay in USA for only a short period of time (4-8 weeks)?
6. The Patriot Exchange Program notes that J2s eligible to purchase only if the J1 holder is insured with the same plan. is it saying that my family members must all buy insurance from this specific progrm when I have selected this?
7. How can I purchase them?

Sorry for so many questions, I will appreciate it if you could answer me point by point.

Thank you so much for your rapid reply. The insurance plan provided by USF itself is too expensive for my family to afford it (around $2400 for J1 and 4000 for J2). Is the Patriot exchange program suitable for my daughter, she is only 5-years old.

Below is the USF insurance requirement, which is described in DS2019 request form :
For health insurance, you and your family can either use USF’s plan, which covers the mandate for J visa holders, or if you use your own insurance, you can have the company complete a compliance form which we will send you with the DS2019.

Health Insurance Requirement
Immigration regulations require that all J exchange visitors have health insurance that meets the following minimum federal requirements:
• Medical benefits of at least $50,000 per accident or illness
• Repatriation of remains in the amount of $7,500
• Expenses associated with the medical evacuation of the exchange visitor to his or her home country in the amount of $10,000
• A deductible not to exceed $500 per accident or illness.

USF Student Health Services insurance policy for international students, graduate students, and postdoctoral fellows meets this requirement and is recommended for use by all USF exchange visitors. The requirements you mentioned are simply standard J visa insurance requirements. Therefore, both the plans you mentioned will meet those requirements. However, I would recommend Patriot Exchange Program as it is a comprehensive coverage unlike Inbound USA that has sub-limits for each and every medical benefit.

Yes, you can buy Patriot Exchange Program for your daughter as long as you buy the same plan for yourself or you buy through your school for yourself.

Hi, I am back again.
The university sent me one insurance compliance form which I should have the company complete to show that my insurance meet the requirements of USF, and I would bring the form with me when I come to check in. Could you please find the attached document?
Another question is how can I buy the insurance in China? I want to settle insurance down before my leaving for USA.

You can get the documents by postal mail. However, they will look exactly the same that you see by email. Now a days, everything is computer printed and no one does hand written signatures on such documents.

In all plans, buying and paying is the same duration. If you want to buy for 12 months, you need to pay for 12 months upfront. For Patriot Exchange Program, in order to renew in future, you must purchase initially for 3 months. After that, you can renew month by month. But if you need the proof that you have the insurance for 12 months, you must pay for 12 months upfront.

I wil study as a Visiting Scholar at Columbia University from September 2014 to September 2015. I will come to New York with my dependents ( wife and a child).
Shoud I do health insurance yearly at the beginning. I wonder If I can obtain health insurance per period one by one (Autumn-Winter-Summer) because of the yearly prices considerably high rather than periodical prices. Should I buy sparately health insurance for them

You can purchase insurance just for yourself or for yourself and your dependents together. It is entirely your choice.

You can either purchase the insurance for few months at a time or you can purchase for the entire year at a time. The total premium would remain the same in either case. However, in some (not all) plans, whenever you renew, there may be $5 renewal fee.
