You can either renew the study usa insurance online or complete a paper renewal notice/application.

- Fill renewal form: You have 2 options:
- If you have received your renewal notice in the
mail, please fill it and send it back to us.
- Otherwise, fill Paper Application. In section 2 of the application, indicate that you are renewing and provide your current policy number.

- Send renewal form:
You can send us the renewal form in either of following ways:- Paying by credit card (MasterCard, Visa or

You can fax us the application, email it to us at , or mail it to us. If emailing or faxing, send it along with the check and you must write 'check by fax' on the check. - Paying by money order:

Fill your personal details.

Either fax it to us at (972) 767-4470 or email it to us at or mail it to us. We will get it filled and send it directly to your school.
