Renewal Procedure - Study USA HealthCareTM


You can either renew the study usa insurance online or complete a paper renewal notice/application.

  1. Renew the insurance:
    • Online Renewal

    • Paper Renewal:
      1. Fill renewal form: You have 2 options:
        1. If you have received your renewal notice in the mail, please fill it and send it back to us.
        2. Otherwise, fill Paper Application. In section 2 of the application, indicate that you are renewing and provide your current policy number.
      2. Send renewal form:
        You can send us the renewal form in either of following ways:
        • Paying by credit card (MasterCard, Visa or Discover):

        • Paying by check:
          You can fax us the application, email it to us at , or mail it to us. If emailing or faxing, send it along with the check and you must write 'check by fax' on the check.
        • Paying by money order:
          You must mail the application to us along with the money order.
  2. Compliance form:
    • Complete the compliance form.


      Fill your personal details.


    • Either fax it to us at (972) 767-4470 or email it to us at or mail it to us. We will get it filled and send it directly to your school.

    • If your school has an online compliance form or if it follows a different procedure, please follow that procedure instead.
