When you go the provider (doctor, hospital etc), you have to ask 'Do you
particpate in < Network NAME FROM ABOVE >?', as the doctors may
not be aware of Diplomat LT (Long Term) or Global Underwriters or even The Insurance Company of State of Pennsylvania or Chartis.
When you go to the providers in the network, they can accept the insurance card and bill the insurance company directly. Also, the network providers
will only charge a Network Negotiated Fee (NNF) or also called Contracted Fee.
However, the plan does not restrict to visiting the providers in the network
only. You can visit any doctor you wish. When you go to the doctor outside
the network, it depends upon the doctor whether to bill the insurance
company directly or not. If the doctor does not accept the card, you
have to first pay and then get reimbursement by filing a claim form.
When you visit the doctor outside the network, there is no network
negotiated fee but there is no reduction of benefits either. Insurance
company will pay according to whatever the doctor charges
are, of course after the deductible is paid, you still have to
pay the co-insurance as applicable.
There is no network of providers for prescription drugs (medicines).
