I will visit USA with my family (my spouse and my son) since the 15 July 2006 to the 31st August 2007 with a J1 visa. I shall be there as an exchange researcher in Oak Ridge National Lab, Oak Ridge, TN. I would like to receive a quotation for a medical insurance for the whole family for the above mentioned period, with a deductible of c.a. 100 $ and fulfilling the conditions of a J1 visitor.
