I am looking for a plan that would transfer him to my choice of hospital if surgery is needed in the US. Is there a plan available like that?

To the best of my knowledge, no such plan exists.

Keeping in mind the sick/injured person's health and safety, It is not practical to fly to the place of your choice. It is very important to take the patient to nearest facility first.
